Preregister Online
You may preregister with our office by filling out our secure online patient registration form. When you arrive at our office during your first visit, we will have your completed form available for your signature.
Prefer not to preregister?
No problem. Just arrive thirty minutes prior to your appointment to complete the paperwork.
Once You Arrive
When you arrive for your appointment, please be sure to bring the following:

A list of medications

Your insurance information

A panoramic x-ray:
If you have an x-ray that was taken within the last year, please ask your dentist/orthodontist to provide. If not, we will take a digital panoramic x-ray during your initial visit.
