Scheduling an appointment is easy.
During your first appointment, we will learn more about your health history, take and review x-rays, do an oral exam, and discuss your treatment options.
Before you arrive, you will be asked to about to provide:
New patient registration information
Prior to your visit, we ask that you complete your New Patient Registration paperwork. This includes your contact and insurance information and a list of your current medications. If you’re unable to fill this out online, please arrive 30-minute prior to your appointment to complete the paperwork.
A panoramic x-ray
If you do not have an x-ray to provide from your dentist/orthodontist that was taken within the last year, we will take a digital panoramic x-ray during your initial visit.
